Mis au point dans les années 90, les stents constituent l'un des grands progrès techniques de la chirurgie cardiologique et vasculaire. Ces petits ressorts métalliques permettant d'éviter qu'une artère ne se bouche complètent aujourd'hui grand nombre d'angioplasties, intervention visant à déboucher les artères.

Qu'est-ce que la pose d'un stent ?

Comme pour une angioplastie, le stent est monté sur un minuscule ballonnet que l'on gonfle à l'intérieur de l'artère coronaire pour repousser les plaques et rétablir le flux sanguin. Une fois les plaques écrasées contre la paroi de l'artère, le stent constitue un échafaudage miniature étayant la paroi de l'artère.

Pourquoi un pontage plutôt qu'un stent ?

Une étude sur 800 patients fait notamment ressortir que le pontage coronarien diminue plus nettement les risques d'infarctus.

Quel médicament après pose de stent ?

Le traitement anti-agrégant standard après pose de stent actif coronaire est une bithérapie aspirine + inhibiteur du P2Y12 pendant un an, suivie de l'aspirine seule. Il a pour objectif de diminuer le risque thrombotique, mais la bithérapie anti-agrégante est associée à un risque de saignement.
